Why am I receiving this bill?
Your provider has partnered with Phytest to have all laboratory charges billed separately from their services.
Where is my patient ID?

Your patient ID number is located on the left side of your bill highlighted in green.

What forms of payment are accepted?

We accept Visa, Mastercard, Discover, Amex, electronic checks, or physical checks.

Do you offer prompt pay discount?

Yes. All prompt payments are due at the time the offer is given.

Do I pay my provider or Phytest?

All laboratory charges are to be paid directly to Phytest through our payment portal or can be mailed to

How long do I have to make a payment to Phytest?

How do I set up a payment plan?

Please call our patient services team at 855-943-0205 to speak with a live representative to assist with setting up a payment plan.

I called to make a payment early August and then after realizing my insurance did end up paying the bill, I called twice about a refund. All 3 times I talked to Toni and she was AMAZING.
