Why am I receiving this bill?

Your patient ID number is located on the left side of your bill highlighted in green.
What forms of payment are accepted?
We accept Visa, Mastercard, Discover, Amex, electronic checks, or physical checks.
Do you offer prompt pay discount?
Yes. All prompt payments are due at the time the offer is given.
Do I pay my provider or Phytest?
All laboratory charges are to be paid directly to Phytest through our payment portal or can be mailed to
